How to use outcome measures

    Build a template

    Go to Settings → Outcome Measure Templates and add sections, questions and scored options (or a free-text/date question that doesn't score).

    Set the scoring rule

    Choose sum or average scoring and define severity bands (e.g. 0–4 Minimal, 5–9 Mild, 10–14 Moderate) so every score gets a plain-language label.

    Administer it to a patient

    From a patient's profile, open the Outcome Measures tab, pick the template and fill in their answers — the score and severity calculate instantly.

    Track progress

    Administer the same assessment again at a later visit — the patient's Outcome Measures tab automatically charts every score over time.

Because it reuses the same template-builder engine and patient-profile surface as the rest of Quzek, outcome measures feel like a native part of your workflow — not a bolted-on form tool.

Frequently asked questions

Does Quzek come with standardized instruments like PHQ-9 or GAD-7 pre-loaded?
No — Quzek ships the template builder itself, not pre-written copyrighted instrument text, so your practice authors (or transcribes) the exact assessments it uses, with full control over wording and scoring.
Can I use it for a fully custom, non-standardized assessment?
Yes. Any combination of scored and unscored questions works — you're not limited to named clinical instruments.
Where does a patient's score history show up?
On the patient's own profile, under the Outcome Measures tab — every past administration is listed, and a progress chart appears automatically once a template has been administered twice.
Can I turn this module off if my practice doesn't use it?
Yes — Outcome Measures is one of the toggleable modules in Settings → Modules & Profession Pack, on by default for profession packs that typically use standardized assessments.
