Clinical records

Clinical notes & body chart software

Design the exact assessment your discipline needs, then capture structured treatment notes and annotate body charts against each visit — no generic forms, no paper.

Quzek lets you build your own clinical templates from ten field types — text, paragraphs, yes/no, checkboxes, dropdowns, dates, file uploads and interactive body charts — grouped into sections. Attach a template to a service and it becomes the note your practitioners complete for that visit, kept on the client's record.

Notes shaped to your practice

How to use clinical notes

  • 1

    Build a template

Go to Settings → Body Chart Templates and create sections and questions with the field types you need.

  • 2

    Add a body chart

Upload anatomical images to your Body Chart Library so they can be annotated in notes.

  • 3

    Attach to a service

Set the note template on the relevant service so it appears for those appointments.

  • 4

    Record in session

Complete the note on the appointment or client record, annotating the body chart where needed.

Everything you capture stays attached to the client and the visit, in your practice's own isolated database — so your clinical history is complete, structured and private.
